Hospital Forms
We would appreciate it if you would print, fill out and bring with you the form prior to your visit. Filling them out prior to your appointment time and bringing them with you will greatly expedite the registration process. For the forms that you can fill out online there are no other steps you must take.
- New Client Form - Please fill out prior to your first appointment at Mayfair Animal Hospital only.
- New Patient Form - Please fill out prior to your pet’s first appointment only.
- Cat Lifestyle Checklist - Please fill out prior to your cat’s next appointment.
- Dog Lifestyle Checklist - Please fill out prior to your dog’s next appointment.
- Client Update Form - This form is to update your information such as your address, telephone numbers, emergency contact information, etc.
- Therapeutic Refill Form
- Boarding Registration Form - Please fill out one form per pet staying with us. This form includes important information about your pet’s medications, food and your emergency contact numbers.
