Please give us the nature of your requested appointment, and 3 calendar dates that you would be available to come in. For example: "Fido needs his yearly exam and vaccines, and he has been limping. I can come in Nov 5th, Nov 13th, or Nov 15th."

Would you prefer a morning or afternoon appointment, or would you prefer to drop your pet off for the day?
